ZODIAC Uses Zero-Shot Diffusion Priors to Reconstruct Spinal Anatomy Hidden in Intraoperative Ultrasound
Andreas Maier published a detailed thread explaining the new method ZODIAC: spinal procedures such as epidural injections and pedicle screw placement increasingly rely on real-time ultrasound because it is radiation-free and inexpensive. But bone is a nearly perfect reflector of sound, leaving deep acoustic shadows behind each vertebra, so on B-mode ultrasound the vertebrae appear only as bright shells; combined with the narrow field of view of handheld probes, physicians see fragmented 2D slices rather than a continuous 3D spine. Recovering the full lumbar spine shape from sparse, noisy surfaces is an ill-posed problem.

- ZODIAC abandons the traditional supervised training on paired broken/intact scans and instead learns a zero-shot diffusion prior of healthy lumbar spines, completing the parts occluded by acoustic shadows at inference time.
- Technical pipeline: an octree representation of the lumbar spine is first compressed with a VAE trained on 413 meshes, then a diffusion model runs; during diffusion, predictions are blended with noise-added ultrasound point clouds, filling in missing parts while preserving real bone.
- Tested on 2 phantoms and 6 volunteers (including 3 difficult cases): on par with supervised methods on clean data; on the hardest cases the 95% Hausdorff error drops by up to 22% and variability by 80%.

- On limitations, only these are mentioned: validated on just three scanners, F1 drops by about 15%, and all volunteers were healthy; the authors' remarks about performance on real clinical patient data remaining to be verified were cut off in the post.
