ZODIAC Fills in the Spine Where Ultrasound Goes Blind From Bone Shadow

Andreas Maier reviews the new ZODIAC study: intra-operative ultrasound is radiation-free and cheap, but bone reflects nearly all sound, casting deep acoustic shadows behind each vertebra; combined with a handheld probe's narrow field of view, surgeons see disjoint fragments instead of a continuous 3-D spine. Recovering the full lumbar shape from such sparse, noisy surfaces is an ill-posed inverse problem.
Prior work trained supervised models on paired synthetic "broken" scans derived from CT spine models, but simulated occlusion may not match real acoustic shadowing. ZODIAC reframes the question to fill these physics-driven data gaps. Tested on 2 phantoms and 6 volunteers (3 hard cases), it matched supervised methods on clean data, cut 95% Hausdorff error by up to 22% on the hardest cases, and reduced variability by 80%; after seeing only three scans, F1 dropped 15%. All volunteers were healthy.
