Hermes Agent user hit 3-10x daily bug where learned lessons can't be written to user-owned skills

User PixelRainbowNFT reports a high-frequency Hermes Agent pain point: after hard-earned lessons and RCA/fixes, the agent often fails to write them into a skill—it records them to memory, which doesn't make it better next time. This happens 3-10 times per day; the user claims a non-noisy fix and offered a PR.
The linked GitHub issue #134289 details the root cause: when background skill review targets a user-owned skill, skillmanage's backgroundreviewpreflight → backgroundreviewwriteguard refuses the write before the staging gate, even with skills.writeapproval: true, losing the lesson with no surfacing path. Fixed by Teknium's PR #134432.
