The resilient-agent loop: detect failure, recover, re-plan, know when to stop

A robust agent shouldn't just Plan → Act → Repeat. It should run a fuller loop: Plan → Act → Observe → Detect Failure → Recover → Re-plan → Stop/Escalate. The goal isn't agents that never fail, but agents that fail safely, recover intelligently, and know when to stop or call for help.
