Claude Code v2.1.312 fixes large-file overwrites, adds OIDC user_id_claim support

Claude Code v2.1.312 ships several updates:
- New oidc.useridclaim: the Claude apps gateway can now identify users via an idtoken claim other than sub, e.g. Microsoft Entra ID's oid.
- Policy fetch fix: managed settings and organization policy are now correctly fetched for the rest of a session after signing in through the VS Code extension.
- Large-file overwrite fix: Claude previously could overwrite very large CLAUDE.md, rules, or @-included files after seeing only their beginning; Write now instructs Claude to read the whole file or edit in place.
