109 of 529 AI Agent Incidents Involved No Adversary, Arguing Red Teams Must Test Normal Operation
anacondainc · x · 2026-10-26
Anaconda highlights that 109 of 529 documented AI agent incidents involved no adversary at all — systems failing during normal operation, not under attack. With SR 26-2 excluding agentic and generative AI, nobody is systematically testing these everyday failure modes.
An accompanying graphic illustrates three scenarios across banking, insurance, and asset management: a support agent quoting a wire fee absent from disclosures, an insurance agent promising coverage the policy excludes, and an advisor recommending a leveraged fund to someone retiring next year. Each system did exactly what it was built to do — in a situation nobody tested. The authors argue red teaming must cover the full AI workflow, not just adversarial cases.
