81% of engineering leaders hit AI-related production incidents in six months

An Undo.io survey of senior engineering leaders found 81% had a production incident tied to AI coding tools in the past six months; 93% said an AI hallucination sent their team chasing the wrong root cause; 94% lost productivity unpicking AI-generated code; 91% saw test escapes or defects reach production.
An August SauceLabs study found similar patterns: 80% of executives traced an incident to AI-generated code, 83% said AI produces over 10% of production code, yet only 6% test AI-generated code with AI tools most of the time — writing got automated, checking stayed manual, and nobody budgeted for the downstream bottleneck.
Author's advice: before AI writes more than a quarter of production code, put AI-driven verification on the same pipeline — generate tests alongside code, gate merges on coverage, and keep a named human owner for root cause.
