Cloudflare MCP Server now pings agents in real time about Workers issues

- Cloudflare MCP Server can now proactively notify agents of new issues on your Workers, powered by Thomas Ankcorn's new real-time issues feature and the draft MCP Event spec.
- Demo: an agent created a deliberately erroring Worker, subscribed to cloudflare.alert.workersobservabilityrealtimeissue, and received a live issue event (issue details, Worker name, Event ID) within 2m50s while the subscription stayed active.
