OpenCode v1.19.0 ships manual updates and a path to OpenCode 2

OpenCode v1.19.0 is out: opencode upgrade --major moves you to OpenCode 2, updates in the TUI are now manual via /update instead of automatic, the OpenAI ultrafast service tier is supported, and PTY connection origin/ticket validation got fixed. Community contributor @dc85 fixed exo usage attribution and docs.
