Codex Windows elevated sandbox fails when node_repl.exe is running (OS error 32)
john77leblanc · ghdev · 2026-10-09
A developer filed issue #52389 on openai/codex: on Windows with windows.sandbox = "elevated", sandbox setup intermittently fails because the helper can't update the ACL on noderepl.exe while the file is in use by the noderepl MCP runtime, yielding OS error 32 and helperunknownerror: setup refresh had errors.
The author analyzed 28 failed sandbox refreshes over 24 hours — all showed the same file-sharing error, first appearing exactly when noderepl was first used, while 1,060 earlier refreshes were clean. The failure persists with codex exec --ignore-user-config, ruling out user MCP config. Affected versions include npm CLI 0.160.1 and app-server daemon 0.161.0/0.162.0. The issue has 8 comments so far.
