Claude Code v2.1.295 adds blocking hooks, terminal status protocol, and gateway model whitelists

Claude Code v2.1.295 ships notable reliability and ops features:
- onFailure: "block" for command and HTTP hooks: hooks that fail to start, time out, or exit unexpectedly now block the action instead of passing through
- OSC 7501 Program Status Protocol support so terminals can show whether Claude Code is working, waiting, or done
- Claude apps gateway upgrades: optional per-upstream models whitelist (with wildcard support) and timeouts.upstreamttfbms to cap stream start latency across Bedrock, Vertex, Foundry with automatic failover
- /copy picker strips quote markers; plugin install/enable/disable warn on settings files that fail to load
- Audit events now carry upstreamrequestid for support debugging; mods get $.ui.notify for native notifications
