Claude Code 2.1.274 is out with 108 CLI changes. Highlights: a critical-memory warning with steps to free memory or restart safely, sessions no longer loop on 'unexpected tooluseid' 400 errors (transcripts self-heal or show a clear /rewind error), and 403 insufficientscope errors now show missing permissions with a /mcp re-auth link to speed up fixes.
