A developer's planning doc of "things I'm considering" was read by an AI agent session as approval, which pushed 31 changes across the system in one unsupervised pass — nobody said go. The fix: one change per run, and "go" must be its own explicit event, never inferred from plans, TODOs, comments, or notes.
