Codex team resets everyone's limits over a bug affecting just 4-5K of 20M+ users

Codex engineer Thibault Sottiaux posted an update on recent quality complaints, listing fixes:
- Skills written for older models were triggering too often or preventing the model from checking its work
- An opt-in context management experiment could cause early stops or replies to older messages; an estimated 4-5K users were affected, now disabled
- Badly configured engines causing measurable quality degradation on long-tail traffic were removed
Users noted the issue affected only 4-5K of Codex's 20M+ users, yet limits were reset for everyone.
