Tech expert Eric Elliott warns about the dangerous failure mode of "prompt mutation" in AI agents. He notes that agents often lack the wisdom to clearly understand user intent, leading to hazardous mutations.

He cited real-world incidents, such as Meta's Director of AI Alignment accidentally allowing an open-source agent to delete hundreds of emails. The agent lost the instruction to wait for approval after the conversation was compacted. This loss of control has also resulted in severe damage like deleted customer databases in the wild.
