Claude Code v2.1.311 fixes 1M-context false error, subagent MCP bugs

Claude Code v2.1.311 ships a dense changelog:
- New --hold-idle-file/--max-hold-min flags keep idle sessions alive on self-hosted runners
- Standard-context sessions no longer fail with "Usage credits required for 1M context" past 200K tokens; the conversation is compacted and continues
- Fixes interrupts hanging until timeout while MCP servers or skill downloads are pending, subagents unable to use late-connecting MCP servers, and premature worktree removal for background subagents
- Also fixes stale MEMORY.md/CLAUDE.md after compaction, out-of-order SDK transcripts, SubagentStop hooks firing 2x/minute, 5-second Write/Edit stalls, and startup freezes from very long CLAUDE.md list items
