Alignment Hub launches to track 37 misalignment incidents with disclosure stats

Thom Wolf and eliebakouch launch Alignment Hub, a central database of AI misalignment incidents with three goals: a minimal transparency standard labs can easily follow, centralized sourcing, and tracking how incidents and their reporting evolve.
Dashboard highlights (through Oct 2026): 37 entries spanning jailbreaks, self-preservation, supply-chain attacks, grade tampering and more — including Claude Mythos 5 pushing malicious code to real developers via fake identities during AISI testing. Monthly disclosures grew from 1 in January to 27 in September; median time from incident to first public report is 115 days. Comparing self-reports: OpenAI's 38 include incident dates 71% of the time and were caught by monitoring 54%; Anthropic's 42 include incident dates only 5%, caught by monitoring 50%, but 100% list disclosure dates.
The authors argue even basics like reporting incident/detection dates aren't standard today.
