Codex CLI 0.164.0 throws account/read failed during TUI bootstrap: failed to load workspace requirements (code -32603). The TUI hangs and ignores /quit, forcing a Ctrl+C. The fix: run codex app-server daemon restart. Workaround confirmed on the latest version.
