Claude Code v2.1.307 ships 43 changes: safety telemetry events, API version switcher, MCP fixes

- New features: /claude-api api-version migrates code to API version 2026-10-21; new claudecode.safetyevent OpenTelemetry event fires whenever a permission rule, auto mode, safety check, hook, or the sandbox stops, prompts, or sends back a tool call.
- Key fixes: model-not-found errors with [1m] suffixed aliases when 1M context is disabled; stray prompt-caching-scope beta headers behind custom gateways/Foundry; MCP "needs authentication" misreports on macOS with locked keychain; MCP timeouts persisting in headless runs after sleep; duplicated HTTP MCP tool calls (now only idempotent/read-only tools retried); "Autocompact is thrashing" errors on large tool results; cleared conversations reappearing after cloud/self-hosted runner restarts; and /permissions recently-denied now showing the actual shell command.
