Codex desktop bug leaves Work sessions stuck on 'Awaiting approval' with no controls
jasonritchie · ghdev · 2026-10-19
A reproducible bug reported on openai/codex (issue #55123): on macOS (Apple Silicon), ChatGPT Work sessions eventually pause with an "Awaiting approval" status, but the desktop app shows no composer, approval button, or permission controls — making the session effectively read-only.
The user hit this across three separate Work sessions, including one deliberately simplified to test basic research functionality. On ChatGPT Plus, the agent becomes impossible to resume, approve, or redirect once stuck — a notable usability defect for agentic workflows.
