Codex Desktop bug hammers API with 14,637 failed reads as access-denied errors get retried endlessly

A bug report (openai/codex #53677) details how Codex Desktop for macOS treats access-denied hydration errors (-32003 "thread access denied") as retryable, causing the cloud "Your dot"/Orbit catalog to reload metadata for the same 65 chats nonstop.
Key measurements from October 13 (00:17–13:53 UTC):
- 14,637 distinct failed request IDs across 65 chat IDs, with later minute buckets hitting 260 failed reads per minute;
- Fully restarting the app does not stop the traffic — the catalog refresh repeats roughly every minute;
- Versions involved: Desktop 26.1007.30912 (build 20156), bundled app-server 0.162.0-alpha.17.2, macOS 26.6.2 on Apple Silicon.
The reporter pinpoints the root cause as error-classification: a terminal permission failure gets wrapped without an HTTP-style status code and falls into a retry path. The issue extends existing #53178 and suggests consolidation.
