Gemini CLI PR fixes /ide reconnect failures after VS Code window reload

PR #29744 in google-gemini/gemini-cli fixes the client side of /ide enable failing after a VS Code "Developer: Reload Window" (or extension update/disable), where the surviving integrated-terminal CLI stayed "Disconnected" until a full restart.
Two defects in IdeClient: (1) discovery relied on a single port file — after reload, the extension host starts a new server (new port/token/file) while env vars stay frozen, so a leaked stale file was deterministically chosen and connect() never fell through to the live one; (2) superseded clients were never closed and error/close handlers were bound to the client rather than the transport, letting SSE retries against the dead port flip healthy state back to Disconnected. Fix spans two files in packages/core with no public API change.
