Claude Code 2.1.294 fixes instruction-based hooks that failed to block commands and premature agent stops

Claude Code CLI 2.1.294 ships two changes: a fix for instruction-form prompt/agent hooks (e.g. 'Block commands that...') that allowed what they should block, and improved judging of Stop/SubagentStop hooks so Claude is less likely to stop early.
