Agent policy-boundary starter updated with idempotency, audit logging and independent validation

The author updated a small FastAPI/Pydantic reference implementation built around one idea: the model can propose an action, but it should not be the authority that approves it. The new version adds resource-scoped idempotency so retries don't repeat operations, audit logging for ALLOW decisions (not just escalations), and independent validation signals instead of trusting model self-confidence. The flow: typed validation → worker → independent validation → policy boundary → ALLOW / HUMANAPPROVAL / HUMANREVIEW / REWORK. The author is soliciting feedback on which production control to add next (durable workflow state, distributed locking, scoped service identity, rollback/compensation) and real incident examples.
