Re-enable Ultracode in Claude Code's VS Code extension with one settings. line

After a recent Claude Code update, /effort ultracode in the VS Code extension errors out asking for dynamic workflows. The author's working fix:
- CLI: /config → Dynamic workflows → on.
- VS Code extension: edit /.claude/settings. and add "enableWorkflows": true as a top-level key next to "model" (after any "hooks" block, not inside it), then reload the window.
- If still failing: check that the CLAUDECODEDISABLEWORKFLOWS env var isn't set (it overrides the setting); note that some plans/orgs disallow workflows or xhigh effort entirely.
Verified on extension v2.1.241, Windows 11.
