Wire an eval gate into GitHub Actions so prompt regressions die before merge

- The problem: prompt changes ship exactly like code changes, but nobody measures either — a bad prompt reaches prod the same way a good one does.
- The fix: an eval gate in CI — run evals in the pipeline and block merges on regression, so regressions die in the PR queue, not in production.
