Claude Code bug leaves stale worktreePath metadata after worktree directories are removed

An issue filed on anthropics/claude-code (#95723) reports that getsession and session listings keep returning stale worktreePath/worktreeName metadata after the underlying worktree directory is deleted (e.g. after PR merge and cleanup).
- Reproduced across multiple sessions: paths point to directories confirmed absent, branches deleted locally and on origin; even a scheduling-only session that never touched the codebase shows the field.
- At least one other session correctly lacks the field, so cleanup exists but is inconsistent.
- Low severity (no data loss), but misleading: metadata suggests a session can be resumed into an existing worktree when it would need full recreation or would silently fail.
