Claude Code 2.1.278 moves Auto mode classifier server-side, ending classifier overhead charges

- Key change: Claude Code CLI 2.1.278 defaults Auto mode's safety classifier to run server-side (for Claude API and Enterprise users, plus Bedrock, Vertex, Foundry and gateways), folding it into the session's own model requests so classifier overhead is no longer billed.
- Fallback & notice: CLAUDECODEAUTOMODESERVER=0 opts out on Bedrock/Vertex/Foundry/gateways; when server-side checks can't reach the session, Claude Code keeps using its own classifier requests billed as before and shows a one-time notice before the first checked action.
- New feature: /status now shows an "Auto mode server" row indicating whether the classifier runs server-side (i.e., free of charge).
