asc CLI 5.4.0 makes review submissions fail closed before mutations

The App Store Connect CLI 5.4.0 now fails closed for review submission creation: if discovery, pagination, response envelopes, item inspection, or resource identities are uncertain, asc stops before the mutation. The same preflight and rollback rules now cover background-asset submissions.
