Biomedical world models: a framework for virtual drug trials, intervention design and planning

Researcher Marinka Zitnik lays out how world models apply to biomedicine. Tracing the idea from Kenneth Craik to robot planning, RL, and recent work by Ha & Schmidhuber and LeCun, she contrasts three domains: games (cheap evaluation, Dreamer collecting Minecraft diamonds), robotics (Cosmos and V-JEPA 2 pretrained on video, adapted with little robot data), and driving (GAIA-1 simulating rare scenarios). Biology resembles the latter two — data collection is expensive, interventions rare or unsafe, and measurements often destroy the system. The framework enables simulating treatments on the same virtual patient, reverse-designing interventions toward target states, and sequential treatment planning.
