Claude Code 2.1.267 adds maxEffortLevel cap and system prompt snapshot toggle

Claude Code shipped v2.1.267 just 18 hours after the previous release, growing the bundle by 278 kB. Key changes:
- A new maxEffortLevel setting (top-level or per model under modelSettings) caps effort level across every provider, including Bedrock, Vertex, and Foundry, while users can still pick a lower level
- A --system-prompt-snapshot off flag renders the system prompt fresh on each request instead of reusing the conversation's recorded prompt, useful for iteration and debugging
