Claude Code 2.1.266: how one undocumented env var broke every gateway request

The full changelog for Claude Code CLI 2.1.266 contains one fix: in 2.1.265, the undocumented CLAUDECODEUSEGATEWAY env var—previously only honored when both ANTHROPICBASEURL and ANTHROPICAUTHTOKEN were set—began forcing Cloud-gateway sign-in on its own. Setups pairing it with an API key, apiKeyHelper, or custom auth headers failed every request with "Not signed in to the Cloud gateway". 2.1.266 restores the old behavior; no configuration change needed.
