opentel-mcp v0.11 fixes server-agent trace disconnection via W3C

Released opentel-mcp v0.11.0, which bridges the gap between MCP server telemetry and the agent trace that called it using W3C traceparent propagation. The update also introduces per-model pricing overrides (covering 19 built-in models plus custom rates), adds missing embedding model costs, and fixes a bug where CallToolResult.isError=true inside an HTTP 200 was incorrectly marking spans as successful.
