Claude Code permission prompts kept returning because Auto Mode was never actually persisted

A user says Claude Code keeps interrupting them with repeated “always allow” and “allow once” permission prompts, even after setting default allow rules and editing settings.json.
In the update, they discover the real issue was that Auto Mode was not actually persisted as the default. They also learn that skipAutoPermissionPrompt only records that the opt-in dialog was accepted, and that the allowlist was pinned to old servers. After fixing those settings, they expect the prompts to stop.
