Claude agent reports now expose model fallback instead of failing silently

Every report comment now includes a Model: line.
Examples shown in the thread:
- Model: opus
- Model: sonnet (fallback – configured: opus)
The important change is that if dispatch is unavailable, the work runs inline and the report makes the degradation explicit instead of failing silently.
