Your Agent Can Be 'Up' and Completely Broken at the Same Time

A developer on Reddit shared a story highlighting the deceptive nature of AI agent failures. They shipped an agent that passed all benchmarks with green checks and fast responses. However, a user screenshot revealed the agent confidently providing a completely wrong answer in a well-formatted paragraph.
The author points out that 'it responded' and 'a real person got the right answer' are entirely different metrics, and day-to-day dashboards rarely measure the latter. To address this, they started a newsletter called Ground Truth, focusing on how to truly evaluate agent reliability, why benchmarks lie, and the silent failures in production that no dashboard flags.
