Check Document Conflicts Before Retrieval

This introduces Cross-Document Conflict Detection: before injecting retrieved results into the context, the system first checks for factual conflicts among multiple chunks.
If two pieces of information disagree on the same fact, the model is prompted to consider both sides before answering. The author notes that this change eliminated one of the most common early user issues: the model being highly confident but entirely wrong.
