Incident agent stores only engineer-verified fixes in Hindsight to prevent memory poisoning

A prototype agent investigates production incidents with a Groq model but can only write to Hindsight memory after an engineer verifies the fix. The team explains why the verification gate is enforced in both the route and the memory service, and openly lists unsolved problems including authentication and retiring incorrect memories. Code is on GitHub as a hackathon project.
