Claude Code v2.1.319 adds background MCP tasks, gateway spend tracking

Anthropic shipped Claude Code v2.1.319 with several notable changes:
- Auto mode: with telemetry or feature flags off (including behind LLM gateways), calls the server can't verify are now asked about or denied, with no fallback to the built-in classifier (CLAUDECODEAUTOMODESERVER=0 keeps old behavior; Bedrock/Vertex/Foundry unchanged)
- MCP tasks in terminal: tool calls run as tasks move to background immediately, with results reported back on completion; CLAUDECODEDISABLEMCPTASKBACKGROUND opts out
- Permissions: new permissions.denyInsteadOfPrompting denies calls that would wait on a permission prompt and continues the turn, across terminal sessions, subagents and teammates
- New command: /claude-api latency-optimize finds latency hotspots in your Claude API code and tries faster settings against your eval
- Usage & gateway: /usage now shows daily/weekly/monthly spend for gateway users; the Claude apps gateway gains pooled budget priority rules, login notices, and JSON log format
