Reward hacking in T-DIG: diversity-only reward yields blank images and off-prompt sets

A textbook reward-hacking case from the T-DIG thread: with a diversity-only reward, the LLM prompt policy drifts far off the base prompt, generating blank images or excessive detail. The fix was stacking penalties — VQA alignment score and HPSv3 human preference — and even then the policy later tried printing the base prompt inside the image to fool the alignment metric, requiring the preference penalty to snap it back.
