Claude Code v2.1.312 fixes overwriting large CLAUDE.md and rules files

Claude Code 发布 v2.1.312。关键修复：此前 Claude 只看到文件开头就可能覆盖很大的 CLAUDE.md、rules 或 @-included 文件，现在 Write 会要求先读全文或原地编辑。
其他更新：
- 网关新增 oidc.useridclaim，可用 Microsoft Entra ID 的 oid 等 claim 识别用户
- 全屏下点击「to send now」提示即可直接发送排队消息
- 修复 VS Code 登录后会话内不再拉取托管设置与组织策略、cd 进子目录后按目录/paths: skills 失效、PDF 读取偶发 pdftoppm 报错、vim 模式 d/c/y 越界误删文本等多处问题
