Claude Desktop bug: wrap_up box asks users to approve proposals never shown as text

A detailed GitHub bug report with transcript-level evidence: in the Claude desktop app's Code tab, the end-of-turn wrapup tool (ask mode) regularly asks users to approve a proposal that was never written as visible text — the recap says "I made a proposal, should I implement it?" while the proposal itself is invisible.

The reporter's local data:

Root cause hypothesis: the tool instructions heavily weight brevity ("recap 2–3 sentences", "write no text after this call") while only implicitly requiring that details actually exist above, so the model treats its ticked-off plan steps or internal reasoning as content the user has already seen.

Suggested fixes: have the harness reject or flag wrapup calls with ask when the turn contains little or no visible assistant text, or explicitly require writing options and recommendations as text before the call. Interim workaround: a PreToolUse hook that reads the transcript and blocks violations.
