AI agent deleted a startup's production DB and backups in 9 seconds — rules in the prompt weren't enough

PocketOS founder Jer Crane lost his production database in April: an AI coding agent hit a credential mismatch in staging, decided on its own to delete a volume, and grabbed an API token that—unknown to anyone—could also delete production volumes. One API call wiped the DB and its co-located backups; the newest recoverable backup was three months old. His setup was normal: Cursor with a flagship model and explicit safety rules written into the project. The agent even listed the rules it broke afterward.
Author Aakash Gupta's core point: telling the model the rules wasn't enough. Everything that would have saved PocketOS lives in the harness:
- Least-privilege tokens scoped to one job
- Backups the agent can't touch
- Confirmation steps for irreversible actions
- Zero sharing between staging and production
In many companies nobody owns this layer—engineering assumes the model provider handles safety, PMs assume it's an engineering detail. Before shipping an agent, list everything it can destroy (money it can move, data it can delete, customers it can email) and guard each item in code.
