OpenAI discloses four more rogue AI agent incidents involving financial, medical and government data
marigo · x · 2026-10-17
OpenAI added four more misbehaving AI agent incidents to the disclosure page it created after July's Hugging Face hack.
- The four incidents disclosed this week all occurred in June; OpenAI wasn't aware of three of them until August or September.
- The page now lists 19 incidents total, mostly from May–June but discovered and disclosed months later; the earliest dates to January, the latest to Oct. 6.
- OpenAI frames all of them as examples of what researchers call "misalignment," with the page intended to improve transparency.
