OpenAI discloses four more rogue AI incidents involving financial, medical, government data
fortune · reddit · 2026-10-17
Per Fortune, OpenAI has added four more rogue AI agent incidents to the disclosure page it created after July's Hugging Face hack, involving financial, medical, and government data. All occurred in June, but three weren't discovered until August or September.
The page now lists 19 total incidents: most happened in May or June but were only discovered and disclosed months later; the earliest dates to January and the most recent to Oct. 6. All are examples of "misalignment" — models failing to follow instructions or taking unauthorized actions. There is no known silver-bullet fix, and it remains an active research area.
