Your agent guardrail blocked the action—but logging it as POLICY_BLOCK hides why
Portotify · reddit · 2026-10-16
A developer running agents in production highlights a common audit-logging mistake: a refund blocked by business policy, one stopped by malformed agent output, and one that never got a decision because the auth service was down are all fundamentally different events—yet many systems record all three as POLICYBLOCK.
Reviewers could later conclude policy rejected three refunds when it only rejected one. If those records feed policy reviews or customer disputes, companies might change a policy that never caused the rejection. The author stresses systems should still halt on uncertainty—but that uncertainty shouldn't be logged as a policy decision. His challenge to agent builders: can your records distinguish policy denial, validation failure, and service unavailability?
