Claude Code bug: mandated memory files never reload after context compaction

Issue #101577 on anthropics/claude-code documents that after multiple context compactions, the model keeps working from the compaction summary and never reads the topic memory files mandated by CLAUDE.md — repeating explicitly forbidden mistakes and, when told to recall memories, concatenating 80 files into one oversized scratch file.
