Qwen Code PR adds bounded image and PDF delivery with 768 KiB base64 ceiling

A PR to QwenLM/qwen-code adds bounded image and PDF delivery through the managed runtime provider protocol.
- Each prepared file read uses its own image/PDF capability snapshot for both tool description and actual content delivery; PDFs can return native bytes, extracted text, or rendered pages
- Media reads enforce a 768 KiB aggregate base64 ceiling and an 896 KiB complete tool-result ceiling within the existing 1 MiB envelope; oversized reads settle as explicit FILETOOLARGE tool errors
- Execution workers remain independent of model initialization and don't inherit Omni media uploads; audio and video stay disabled
- Adds regression coverage for capability isolation, byte boundaries, lost replies, and Broker restart recovery
