Codex web bug: 'Always allow' ignored as approval cards repeat and uploads stall

A developer filed an issue on openai/codex documenting a broken approval lifecycle in the web-based dots coding workflow across October 7-9:
- GitHub createblob approval prompts kept reappearing even after repeatedly selecting Always allow; earlier cards showed "This request is no longer pending."
- A 1.3MB Rust archive upload blocked for 1,018 seconds despite a requested one-second yield, with the expected Git object later returning 404.
- A config archive sat pending for 8 minutes, then the original call succeeded without retry — a delayed success rather than permanent failure.
- A small public-corpus upload was denied for an "unverified external repository," yet the same retry with existing authorization succeeded two minutes later.
The author notes not all calls fail and doesn't claim Always allow should override genuine security checks; the core concerns are inconsistent outcomes, unclear approval state, and unrecoverable results from long-running operations.
