opencode PR fixes submission starvation: interactive requests prioritized, queue limit 4→12

This PR fixes opencode's issue where user prompts, agent question replies, permissions, and commands take excessively long to submit or fail with "Timed out waiting for the server" under multi-agent or background task loads.
Key changes:
- Interactive priority queueing: user-facing requests (/prompt, /permission/reply, etc.) are inserted ahead of waiting background sync reads; loopback queue limit raised from 4 to 12 slots, slow-request limit set to 4.
- Targeted timeout budgets: 180s header timeout for priority requests, general timeout raised to 120s, so heavy operations no longer abort active user submissions.
- Resilient form mutations: replyMutation/rejectMutation get 3 retries with exponential backoff (capped at 5s), surviving transient network/queue spikes.
- Database lock contention: SQLite busytimeout bumped from 5s to 30s, plus a 2GB mmapsize to speed page reads and shorten lock durations during concurrent sessions.
- Resilient SSE: connect timeout 2s→10s, idle timeout 45s→90s to prevent premature disconnects.
